Dental Insurance Plans:

• We do not file with any insurance company.

Payment options:

• Cash or Check: Always accepted
• Credit Card: Visa, MasterCard
• CareCredit: Apply online at www.carecredit.com

Please note: Payment is due at the time of service.

We would be happy to provide you with a claim form to submit to your insurance company on your own.  You are responsible for the fees charged by our office, no matter what your insurance coverage may be.
